Empath Personal Care - Help with daily living and support for independence at home
EPIC - HIV services, education, and prevention programs
Home Health - Skilled nursing care and support in the comfort of your home
Hospice - Compassionate end-of-life care for patients and families
PACE (Program of All-Inclusive Care for the Elderly) - Complete care coordination for eligible seniors
Pathways - Ongoing support for individuals not admitted at referral or for those discharged from Empath hospice programs. Services vary by county, and our team will help connect you with the right care close to home.
